n order to be a volunteer with Colorado Palliative & Hospice Care we require that you: • Provide a Health History • Undergo a Criminal Background Check and complete a Criminal Disclosure Statement • Complete routine forms required by Colorado Palliative & Hospice Care • Complete training and a follow-up interview with the Volunteer Coordinator • Direct patient care volunteers will be required to take a TB skin test I certify that the information contained in this application for a volunteer position is correct and complete. I understand that false statements, answers or omissions on this application shall disqualify me from consideration for a volunteer position, or dismissal after placement. I authorize Colorado Palliative & Hospice Care to investigate any or all information on this application concerning my previous employment, education, health, police and governmental records. I hereby release any former employers, educational institutions, police and governmental agencies from all liability for any damages in furnishing information. My volunteer placement will be based on the receipt of satisfactory information and on my ability to perform the essential functions of the volunteer position for which I have applied.
